Five Questions Every Man Over 40 Should Ask His Doctor But Probably Isn't

Most primary care visits do not have time for proactive men's health. Here are five questions worth asking.

The average primary care visit runs about 18 minutes. In that window a physician is managing your immediate concerns, reviewing medications, updating records, and handling referrals. There is not much room for the proactive conversation that men's health optimization actually requires.


Here are five questions worth raising.


First: What is my free testosterone, not just my total? Free testosterone, the biologically active fraction, is what matters for how you actually feel. Ask specifically for free T and SHBG.


Second: What does my estradiol look like? Estradiol rises in men through aromatization and directly affects energy, mood, libido, and body composition. Most standard male panels do not include it unless you ask.


Third: Has my thyroid been fully evaluated? A TSH alone is not a complete thyroid panel. Free T3, Free T4, and thyroid antibodies tell a significantly more complete story, particularly if fatigue and weight changes are part of your symptoms.


Fourth: What are my fasting insulin and HbA1c? Insulin resistance often precedes metabolic syndrome by years and affects hormonal balance in ways that go undetected until significant.


Fifth: Is there a reason I feel this way that has not been identified yet? This is the permission question that opens the door to a more complete clinical conversation.
